Changing Your Contact Details
Update your details
To update details like your name, address, phone numbers or email address:
- fill out a change of contact details form
- phone or visit the surgery during our opening hours
Change of address
If you have moved to a new address, you will need to tell us. You will not necessarily be asked to move surgery but may need to be registered as an out-of-area patient if you no longer fall within our catchment area.
This occurs in a few different areas.
Changing your phone number or email address
You can change these yourself on the NHS App
Changing your date of birth or name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate or deed poll.
If your date of birth has been recorded incorrectly, we will need to see a copy of your birth certificate before we can change it.
