Wellbeing Service
Tiverton Wellbeing
At Amicus Health, we are lucky to have in-house Social Prescribers who can provide personalised one-to-one support for our patients. If you're going through a tough time, Tiverton Wellbeing is here to listen and provide practical advice. The team works collaboratively with GPs and other healthcare professionals, community and voluntary organisations, as well as other service providers in the area.
We can signpost you to services that can help with:
- Employment, education, training and volunteering
- Housing, benefits and financial support advice
- Healthy living and weight loss
- Therapy and support groups
- Social, hobbies and interest groups
How does it work?
- At your appointment, we will help you explore what is important to your health and wellbeing and talk about any challenges you may be facing.
- You will have plenty of time to talk about what matters to you. Everything we discuss will remain confidential, unless we have serious concerns about your ability to keep yourself safe, or we feel you may pose a risk to others. We would always talk to you about this first.
- Your Social Prescriber may suggest things you could do to help improve your situation. They may tell you about local activities, services and organisations who could provide additional support.
- The Social Prescriber will also help to identify ways you can help yourself in the future.
How do I access the service?
You can ask for support from us by referring yourself via a website called Joy.
Visit the website, then click 'Make a referral'. You could also be referred to us by your GP or by another organisation.
Alternatively, call us on 01884 210438.
What do our patients say?
"[My Social Prescriber] was very helpful and found some very good things for me to watch or read. I was very low when we first started talking and I feel a lot better having had the help and support from her. It was just so lovely to unload a lot of stuff without any judgement and I knew it would go no further. Thank you so much"
